PARENTS OF PRE-K STUDENTS
It is time to plan for your preschool student’s immunizations prior to kindergarten entry. The Utah Department of Health has established the immunization requirements for the 2019-20 school year. We encourage you to have these immunizations completed and written verification back to the school before the end of the current school year.
To attend kindergarten, a student must have written proof of receiving the following immunizations:
- 5 Dtap/DT (4 doses of DTaP if the 4th dose given on/after the 4th birthday)
- 4 Polio*(3 doses if 3rd dose was given on/after the 4th birthday)
- 2 Measles, Mumps, Rubella
- 3 Hepatitis B
- 2 Hepatitis A
- 2 Varicella (chickenpox) –history of disease acceptable, must submit a document signed by a healthcare provider to the school as proof of immunity
For children whose parents claim an immunization exemption for medical, religious, or personal reasons, an immunization exemption form must be competed online and a certificate provided to the school. Please go to www.immunize-utah.org to complete the online immunization education module.
The Summit County Health Department is available on Tuesdays from 2:00-4:00 pm for immunization clinic. Please call 435 336-3234 to see if you qualify for immunization services, or you may wish to use your own healthcare provider.
Parents of 6th GradersParents and guardians of 6th grade students should receive a letter from the Middle School reminding them of the immunizations their child will need for seventh grade entry.Students who are not immunized prior to the first day of their 7th grade will not be allowed to stay in school until their immunizations are verified.For 7th grade entry students must have verification of 1) Tdap Booster, 1) Meningoccal vaccine.
